Mee Memorial Healthcare System welcomes referrals from physicians, healthcare providers, medical groups, hospitals, and outside clinics for outpatient specialty and rehabilitation services.

Our team is committed to making the referral process simple and efficient while supporting timely access to care and coordination with referring providers.

How to Submit a Referral

Referrals may be submitted by fax to the appropriate clinic or department.

Please include the following information:

• Patient name, date of birth, and contact information
• Patient insurance information
• Referring provider's name and contact information
• Referral order, including requested specialty or service and reason for referral
• Relevant clinical notes, test results, imaging reports, or other supporting documentation
• Authorization information, if required by the patient's insurance plan
• Providing complete information helps our team process referrals efficiently and avoid unnecessary delays in scheduling.
